Accommodation Booking Form

The Hotel Network on behalf of CAPIG has organised special reduced rates at the hotels listed below. Please make all accommodation reservations and amendments through The Hotel Network. Conference hotel accommodation must be booked on this form to obtain the special conference rates. Room reservations for accommodation must be accompanied by a deposit equivalent to one nights accommodation, or preferably, quote a personal credit card number which we can pass onto the hotel to secure your room. A limited number of rooms have been booked at each hotel so please book early to ensure your first choice is available.

All rates are per room per night, room only unless otherwise specified. Rates are inclusive of GST.

Please enter 0 in the TOTAL ACCOMMODATION COST column if you are using your personal credit card and would like us to pass your card details direct to the hotel to secure your booking. You will be asked to present your credit card on arrival – no payment will be deducted by us.

If using a 3rd party credit card please indicate the amount to process in the above cost column.

Cheque payments will be accepted but must be received at least 7 days prior to your arrival otherwise your accommodation will remain unpaid at the hotel. We can only pass on monies that we have received. Please indicate in the above cost column the amount of the cheque you will be sending. Thank you

Cancellations & refunds: Cancellation of accommodation made after 26th September 2008 may attract the full stay’s tariff. However, transfer of accommodation booking to another person will be accepted. All accommodation cancellations must be notified in writing to The Hotel Network.
